Rates ~ Currently Accepting New Patients

The fee for first Intake Session is $120 for 60 minutes, $150 for 90 minutes. The fee for every session thereafter is $110 for 60 minutes. The amount due may alter if using an insurance provider for payment services (see Assignments of Benefits form and page 8) Payment is due at the time of service. Acceptable forms of payment are: exact-amount cash , check (insufficient-funds checks will be returned upon full payment of the original amount plus $100 for any returned check), or credit/debit card.

The clinician reserves the right to terminate the counseling relationship if more than 2 sessions are missed without proper notification.

The clinician charges his/her hourly rate in quarter hours for phone calls over 10 minutes in length, at length email correspondence, reading assessments or evaluations, writing assessments or letters, and collaborating with necessary professionals (with your permission) for continuity of care. All costs for services outside of session will be billed via the credit card on file PRIOR to responded or completing such form/correspondence.

Insurance
Services may be covered in full or in part by your health insurance or employee benefit plan. If you have insurance as a PPO, you may check with your insurance to see if you can file a claim as "Out Of Network".

United Health Care (Optum) & UMR

Please check your coverage carefully by asking the following questions:

• Do I have mental health insurance benefits?
• What is my deductible and has it been met?
• How many sessions per year does my health insurance cover?
• What is the coverage amount per therapy session?
• Is approval required from my primary care physician?

Reduced Fee
Reduced fee services are available on a limited basis.

Cancellation Policy
If you do not show up for your scheduled therapy appointment, and you have not notified us at least 24 hours in advance, you will be required to pay $75. If you are 15 minutes late to an appointment, your appointment is considered cancelled and you will be automatically charged $75.
